Consent to Participate: I hereby consent to said child’s participation in this event.
Release Regarding Participation:
On behalf of myself and my child, I also agree to release and discharge, and agree to indemnify and hold harmless, Christ Church and its leadership, employees, volunteers, leaders, chaperones, and/or any other persons or entities acting on hits behalf, from any and all claims, rights of action and causes of action that may have arising in the past or may arise in the future, directly, or indirectly from my child’s participation in said event, including the transportation of my child.
Consent Regarding First Aid and Medical Treatment:
Regarding said event, in case of health problem or emergency, I consent to having Christ Church and its leadership, employees, volunteers, leaders, chaperones, and any/or other persons or entities acting on its behalf to administer first aid, and where necessary, to transport my child to the nearest hospital emergency room if needed and to order x-rays, routine tests, and treatment, and to release any records necessary for insurance purposes. In the event I cannot be reached in an emergency, I consent for the physician selected by Christ Church and its leadership, employees, volunteers, leaders, chaperones, and/or any other person acting on its behalf to secure and administer treatment, including hospitalization, for my child.
Release Regarding First Aid and Obtaining Medical Treatment:
On behalf of myself and my child, I also agree to release and discharge, and agree to indemnify and hold harmless, Christ Church and its leadership, employees, volunteers, leaders, chaperones, and/or any other persons or entities acting on hits behalf, from any and all claims, rights of action and causes of action that may have arising in the past or may arise in the future, directly, or indirectly from any first aid treatment, medical treatment, recommendation, transportation, or any lack thereof, provided to my child, with respect to said event. I have read and agree to the Consent and Release provisions above.
